The goal of CNC is to enhance independence and cater to one's lifestyle within the safety and familiarity of their own environment, making it a highly recommended form of care.

We specialize in supporting clients during hospital discharge and liaison processes, ensuring a smooth transition back to their homes. Additionally, we have can develop our own clinical assessments to tailor care plans specifically to each client’s needs.

To maintain optimal health and well-being, we conduct regular check-ups with clients after their hospital discharge, offering ongoing support and monitoring to promote recovery and independence.
